DAP Program and Health Information Exchange
The Arizona Health Care Cost Containment (AHCCCS) Differential Adjusted Payment (DAP) aims to distinguish providers that have committed to supporting designated actions that improve patients’ care experience, improve members’ health and reduce cost of care growth. An integral part of the DAP program includes participation in and meeting milestones related to a qualifying Health Information Exchange (HIE) organization such as Contexture, the umbrella organization for Health Current, Arizona’s sole qualifying HIE.

LETTERS OF INTENT (LOI) & SDOH REGISTRATION FORMS/SPREADSHEETS – DUE DATE: APRIL 1, 2022
Eligible providers must submit a Letter of Intent (LOI) for each DAP Program provider type they wish to participate in. An LOI is required in order to participate in the program. If you intend to participate, please complete and return the appropriate LOI template(s) below and send to DAP@healthcurrent.org by April 1, 2022.
Provider Types who are eligible to participate in the Social Determinants of Health (SDOH) HIE Incentive must complete and return the SDOH Closed-Loop Referral (CLR) Form and SDOH CLR Spreadsheet below and send to DAP@healthcurrent.org by April 1, 2022.
If you have questions about the DAP Program for CYE 2023 as it relates to HIE milestones or SDOH milestones or if your organization has not yet signed a Health Current Participation Agreement, please contact recruitment@healthcurrent.org.
