





A Health Current Participant is a healthcare organization that has executed a participation agreement with Health Current, enabling the organization to send and receive clinical data through Health Current. For example, an organization such as a health system may include multiple facilities or entities but is considered one Health Current Participant. When an organization joins Health Current, a key part of the on-boarding process, with assistance from Health Current, includes a self-selection of one of the following ten participant categories below.

Health Current is an active participant in The Patient Centered Data Home™ (PCDH) initiative. PCDH is a cost-effective, scalable method of exchanging patient data among health information exchanges (HIEs). It’s based on triggering episode alerts, which notify providers a care event has occurred outside of the patient’s “home” HIE, and confirms the availability and the specific location of the clinical data, enabling providers to initiate a simple query to access real-time information across state and regional lines and the care continuum.
