The AzHDR is a free system that will provide a reliable and safe place to complete, upload, store and make accessible Arizonans’ advance directives so end-of-life care will be guided by one’s wishes. The secure and confidential registry provides peace of mind to registrants and their loved ones and offers easy access to healthcare providers.
As the registry moves from its current location within the Secretary of State’s office to Health Current, the AzHDR will allow for seamless, real-time access to advance care planning documents by healthcare professionals and first responders.
